HELP provides referrals to assisted living facilities and nursing homes for Douglas/Sarpy-county residents, regardless of income and including those on Medicaid. From start to finish, we help identify your needs and then find care communities or services to match those needs.
How to Get Help
Step 1
Call 402-341-6559 x102 to make an appointment with Daniel Haynes.
Step 2
At the appointment, fill out a needs assessment form and discuss care options matched to your or your loved one’s needs.
Step 3
Options may include a doctor’s evaluation; in-home services, or long-term care communities. Nursing centers provide ‘round the clock care, while assisted living offers non-medical, daily basic care for meals, bathing, etc.
Step 4
Arrange tours of care facilities. HELP can accompany you on tours and provide assistance to fill out Medicaid forms.
